    Have you ever or do you currently experience any of the following?

    Allergies and hay fever may cause retention issues from watery and rubbing eyes. Which means you may need refills sooner rather than later eg 2wks instead of 3wks.
    You may experience watery eyes during the treatment if you have sensitive eyes.
    If you are pregnant, please make sure you are still able to lay comfortable on your back. Please note that pregnant clients may experience retention issues or an allergic reaction due to fluctuating hormones.
    Please be sure you can lay on your back comfortably for over an hour.
    If you wear contact lenses please make sure to remove them prior to your appointment.
    Eyelash extensions may not be suitable if you are claustrophobic as you will have your eyes closed for over an hour with your bottom lashes taped down and you will not be able to open them until the treatment is complete.
    Oily hair and skin may cause retention issues. Which means you may need refills sooner rather than later eg 2wks instead of 3wks.

    Eye drops may cause retention issues.
    If you have recently experienced conjunctivitis or a sty, please make sure it has 110% cleared up at least 1 week prior to your appointment.
    Eyelash extensions treatment may not be carried out with this condition for safety reasons.

    Have you ever had an allergic reaction or any type of reaction to eyelash extensions in the past?
    If you agree to before and after photos they may be used on our website and social media pages.

    Terms & Conditions 

    * I agree that the above information is true to the best of my knowledge and will inform my eyelash technician if any of these conditions change.
    * I agree to have an eyelash extensions treatment performed on me and understand that proper aftercare is crucial to the longevity and health of my natural lashes.
    * I understand that failure to follow proper aftercare may result in loss and damage to the natural lashes which the eyelash technician is not responsible for.
    * I understand that eyelash extensions can only be 2-3mm longer than my natural lashes, any longer will be too heavy and cause breakage and damage to the natural lash. My eyelash technician will recommend the best suitable length for my natural lashes.
    * In the event of sensitivity or an allergic reaction I agree to let my eyelash technician know ASAP.
    * In the unlikely event you experience a significant amount of lash loss within the first 3 days of application I agree to let my eyelash technician know within those first 3 days so suitable measures can take place.
    * I agree that no refunds will be given once application is complete.
    * During the treatment I agree that I will not open my eyes, make any sudden movement, touch eyes or under eye pads, chew gum, keep my body and eyes as still as possible, will not squeeze eyes together or anything else that could result in injury to my eyes.
